The Guilford Town Clerk and Clerk Assistant are authorized to issue certified and noncertified copies of vital records for all births and deaths in the state of Vermont, and Marriage Certificates specifically for licenses acquired or recorded in the Town of Guilford, VT.

  • Certified copies (Useful for Legal Purposes) are printed on State engraved security paper with raised seal, costing $10 per certificate.
  • Noncertified copies (Useful for Informational and File purpose only) are printed on plain white paper, costing $1 per page . 

Birth and death records from January 1909 to present, marriages, civil unions, divorces, and dissolutions may also be ordered from The Department of Health. Vital Records may also be ordered from the Vermont State Archives and Records Administration.  

To receive a Certified vital records for births, death or marriage from the Town of Guilford in person you must:

  • Have a valid form of Identification (Drivers License or Passport)
  • Fill out a Birth/Death Request Form or Marriage Request Form, either ahead of time or in our office
  • Pay the $10 fee per certificate (Cash or check addressed to “Town of Guilford”)

To receive a Certified vital records for births, death or marriage from the Town of Guilford via mail you must:

  • Provide a copy valid form of Identification (Drivers License or Passport)
  • Fill out a Birth/Death Request Form or Marriage Request Form
  • A check or money order addressed to “Town of Guilford” for the appropriate amount.

      • If you pay online please provide a copy of your receipt.
